Documenting findings, actions, and outcomes

Beginning with the initial installation of a network server or host, comprehensive documentation should be as much a part of a system as any of its hardware and software. The documentation of individual computers and network and system-attached devices should record its repair and maintenance history. This record must detail any troubleshooting activities, successful or not, any repairs, upgrades, patches, fixes, additions to, and even removals from, each system.
