Creating forms is one of the most common tasks that web developers are faced with on a regular basis. concrete5 provides the Form helper to make creating forms easy. In this recipe, we will create a feedback form using the Form helper.

Getting ready

The code in this recipe will be inserted into a <form> element on our website. See the complete example code on the website for this book. Our form will contain the following fields:

Name (text input)
Email Address (text input)
Reason for Contacting (select box)
Message (textarea)

How to do it...

Create a new single page by adding a new file located at /single_pages/example_form.php.
Install the single page by visiting /dashboard/pages/single on your concrete5 website. Enter example_form as the path for the new page.
Load the Form helper:
